how would the top one end up like that?

years of sitting with the clutch down ,just wears the carbon ring away,,, eventually

it was found that I didn't know there were 2 locating 'dimples' in the clutch fork for the bearing. so I removed the fork to fit them properly.

what must have happened was as it was loose, it didn't disengage the gearbox from the engine correctly when trying to change gear.
